Received cost mismatch

We have recently identified an issue where the received cost does not match the PO line cost. We are a multi org set up and the issue is affecting all of the organizations. The received cost can be higher or lower that the line cost and occasionally is $0.00. I have looked for any consistency in the error and have not found any similarity for time, user or cost difference. We are not using invoicing to make any adjustments so I ruled that out as well. reviewing the records shows that the issue has been going on for a while but is random enough that it has not surfaced until recently.
Has anyone else run across similar issues? We are looking for any suggestions on where to look for a solution.

We have run into this (we're on Maximo 6.2.7)
This happens with us when users receive a line item and then change PO line information (unit cost/quantity) then receive some more(or return - or any combination of this).
Instead of undoing receipts, changing the line info, and then redoing receipts

We only have one site ( in Japan) using exchange rates and have not noticed any issues for that site.

We checked on that and have not found that to be the case. The majority of the receipts are for direct issue and are often for a single item.

Try to check/compare the exchange rate at the time the PO is approved and at time PO Line Item is received.

It might be something like this:
IZ28988: WHEN RECEIVING, THE TOTAL RECEIVED COST IS DIFFERENT FROM THE TOTAL ORDERED COS. THIS IS A DUPLICATED ISSUE OF 94647

We are on 7.1.1.6 and have logged several issues in receipting app with IBM. Here is a list of them:
- Incorrect RECEIVEDTOTALCOST calculation for inventory lines of PO in foreign currency (Service Request 03284 999 616)
- Incorrect LineCost when Service receipts entered several times without save for the same PO line (Service Request 04126 999 616)
- PO Receiving application incorrectly process Lump Sum Amount service lines for currencies other then base currency (Service Request 87909 999 616)
- PO Receiving application incorrectly process Lump Sum Amount service lines (Service Request 87447 999 616) - different problem from 87909
Most of them have been resolved in next Maximo releases but how many new were created...
Let me know if you need more details of our issues.
As you can see there are lots what can go wrong...
