Loading PMs through CSV file : Unparseable Date/Time Error

Hi All,
I am trying to import preview one of my PM data load templates for PM.
I get the following error for the Nextdate field :
BMXAA4144E - The date/time format is not valid.
Unparseable date: "2/23/2016 12:00 AM"
Tried all different data/time formats. Seems like none of the formats work.
Note: I have the MM/dd/yyyy format for changedate and it doesn't prompts any error for that changedate format.
Any help or insight would be appreciated.

Have you tried UTC date format such as:
2016-02-23T00:00:00-05:00
I usually find the best approach is to export some data then copy the
format for the import.

Another question, how to convert the standard excel date/time in UTC format. The time zone is US/PST.

Updates :
I resolved the date error but I am getting a new error now :
BMXAA1702E - Cannot read from the file.
null

Excel has functions to extract the date, month, year etc that you can then concatenate to form a UTC string - if there isn't a UTC date format.
