How do you setup the cause and remedy list. When I click on new row the lookup is a required field but there nothing in the list. Do I need to setup a DOMAIN first? What about the valuelist for problems. It already existed and I do not see a DOMAIN being referenced in DB configuration. What am I missing?

Assuming you have already entered and saved the failure and problem codes, with a problem code selected click new row in the causes table window. I would expect other failure/problem/cause codes to be in the select value list, however if there are none or the value you need is not in the list you can enter the value by typing it into the field. Continue to build out each cause for the different problem codes as needed. Then you will select each cause as you build out the remedies. The causes and remedies are built in the Failure Code application and you do not need to setup a domain for them.
I'm using 7.5.0.2, but I remember it being very similar in 6x.
